Sex and Nationality | Candidates have to darken the relevant circles. |
Examination Centre | Candidates will have to choose three exam centres out of the list given with the appropriate codes. They should also darken the relevant circles. |
Place of residence | Candidates living in cities where the metropolitan board, municipal corporation/council function and those who live in District/Tehsil should write ‘urban’. All other candidates should write ‘rural’. |

Candidate’s photograph | Candidates need to paste their recent colour passport size photo with a light background in the appropriate space. The photographs must be pasted and not pinned or stapled. |
Qualifying examination details | Candidates have to write the details of SSC/Class X and HSC /Class XII examination. They will need to write their board name, college name, year of passing/appearing, marks/grade secured in the respective examination, and roll number. |
Declaration by the candidate | Candidates will have to sign the declaration in the given space. Parents will also have to provide their countersign in the designated place. |
Attestation by the Head of Institution | Candidates need to get their application form attested by the Head of Institution where the candidate is appearing in class XII/HSC or has appeared in. |
