Class 12
It should not matter much whichever name you use in the application. But just to be precise, use the name as in the 12th marksheet. And if the "last name" column in the application is mandatory to fill, then fill the name as per 10th certificate. If last name is optional, it's your choice to fill which of the two names.
Generally, the university does not ask for your mother's aadhar card.

As you have not mentioned the name of exam for which you have forgotten application number. But in general you can get your application number through the challan receipt for which you have done the payment of application. Or you can retrieve it by checking the email ID through which you have registered yourself for application. You must have received any message or email and that will contain the application number.

After passing your 12th exam, you can appear for the Indian Maritime University Common Entrance Test (IMU CET) or the All India Merchant Navy Entrance Exam. B.Tech in Marine Engineering course is normally non sponsored and shipping companies visits campus of the institute for placement of Engine Cadets.
MEDICAL FITNESS :
Eye vision (unaided 0.5 (6/12) in each eye or 0.67 (6/9) in better eye and 0.33 (6/18) in other eye with no color blindness.
